I know how to do this, if you are with Vodafone (New Zealand). So sorry if this doesn't help. Please tell me if it doesn't.
1.Firstly, you own to call Vodafone Customer Service (777) from another phone - use your home phone so it doesn't cost much.
2.Secondly, follow the prompts until you find one that says something like "Talk to a supervisor".
3. Thirdly, converse to the supervisor and make sure that they know what your mobile number is and what type of phone it is.
Usually, after you tell the person your phone number, they will transmit you your PUK code. I suggest writing this down somewhere, in case you ever need it again (I'm pretty sure it's merely one code per person).
If this doesn't work, call again.
